We kindly request your permission to use action photos or video footage of the participant for the above stated purposes. Please note that:

• The participant's dignity, privacy, and safety are our priority.

• No sensitive or identifying personal details (e.g., full name, address, NDIS number) will be disclosed alongside the images.

• Images will not be used for any other purpose than promoting or marketing the services and values of our organisation.
